In terms of restaurants the best Ive ever had is at Ditka's in Chicago...for LA Id say the best is at Kate Mantalini's in Beverly Hills or Cheesecake Factory.

If you are in the area, The Pantry had a killer one last time I had it.
Years ago, I had some frozen/microwavable meatloaf from Trader Joe's that was dang good for frozen, but eh.... Not sure if it's still available.
Thre is a place out in the Montclair area that is supposed to have huge portioned, delicious meat loaf. I'll get the name from a guy at work in the morning.

Claim Jumper has delicious meatloaf. You can also buy it at your local supermarket.
I second Claim Jumper. I've never had the frozen but the restaurant's kicks a**!

For fast food, Boston Market's meatloaf is pretty damn good. Not great, but it gives me that comfort food feeling. Claim Jumper is good as well. Another place that I've had great meatloaf on several occasions is at Ariel's Grotto at Disney's California Adventure Park. Not saying its worth getting into the park just to have it, but if you're there, I would say it is the best restaurant meatloaf I've had.

Haven't made it in years, and it was more of a tossing of stuff together rather than an actual recipe.
Basics:
Hamburger 1lb
Sausage 1lb
Catsup used to add moisture
Eggs (2) helps bind the loaf
Bread Crumbs
Onion (chopped)
seasoning to meet your needs Lawry's Seasoning Salt is good, but you can use other things.
I would usually throw a couple of strips of bacon on top with some sauce (usually depended what I had on hand)
Bake at 350 degrees for about 1 to 1/12 hours. Make sure it is cooked all the way through.
You can use more or less meat depending how many you want to serve, and you can also use ground turkey and sausage if you prefer.

My mom likes using the pre-mixed meatloaf stuff from the butcher counter at Stater Bros. It's already seasoned and all you have to do is make it into a meatloaf shape. She'll also use that for her filling if she makes stuffed bell peppers.
Edit: For a kick my dad likes using Heinz Chili Sauce instead of ketchup.
Trader Joe's Italian Turkey Meatloaf is a staple in my home. Damn tasty.

My Grandma (RIP) used to make meatloaf and macaroni & cheese as a staple dinner when i'd visit her. Not in the same dish but they both cook at the same temp and roughly the same time.
